Work Environment

This position is predominantly remote, providing flexibility while allowing for effective communication and collaboration with the team. Key Responsibilities
  • Collaborate seamlessly with multidisciplinary teams including physicians, nurses, and case managers to provide comprehensive and individualized patient care.
  • Conduct thorough psychological assessments and evaluations to identify client needs and develop effective, evidence-based treatment plans that are tailored to each individual.
  • Engage with clients and their families to provide emotional support, educational resources, and critical coping strategies during periods of transition and grief.
  • Actively participate in case meetings, presenting findings and recommendations while integrating input from other healthcare professionals to ensure cohesive care delivery.
  • Stay updated with the latest developments in mental health best practices and contribute ideas for the continuous improvement of service delivery.
Patient Population / Client Focus

The LCSW will primarily work with clients facing complex life challenges associated with advanced illnesses, end-of-life considerations, and bereavement. This role includes providing critical mental health support to both patients and their families, ensuring that emotional needs are addressed sensitively and effectively. Our clients trust us during some of their most vulnerable moments, and it is our duty to equip them with tools for resilience and healing.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • An active Licensed Clinical Social Worker (LCSW) license in Virginia.
  • Proven experience in performing clinical evaluations and developing comprehensive treatment plans, preferably in a healthcare setting.
  • Exceptional interpersonal and communication skills, with the ability to build collaborative relationships within a team dynamic.
  • A strong commitment to patient-centered care and a genuine desire to enhance the mental well-being of every client.
Preferred Experience
  • At least two years of experience in social work, particularly within a hospice or palliative care environment.
  • Experience working with patients and families dealing with grief and loss.
  • Familiarity with electronic health record (EHR) systems for documentation and tracking patient progress.
Compensation and Benefits
