Your Role:

The Clinical Support Nurse - PD Telecare is a Registered Nurse specializing in Peritoneal Dialysis (PD) who provides clinical oversight, remote patient monitoring, and operational support through the PD Telecare program. This nurse plays a critical role in optimizing outcomes for patients performing PD at home by monitoring therapy data, identifying clinical concerns, coordinating follow-up with PD program nurses, and ensuring that patient-centred, evidence-based care is maintained across all virtual channels.

Working collaboratively with PD program nurses, the PD Telecare Nurse supports timely intervention, data integrity, and consistent communication to ensure safe, effective, and high-quality home dialysis management.

What you'll be doing:

Remote Patient Monitoring and Clinical Support

  • Review and interpret Remote Patient Management (RPM) data daily to monitor patient adherence, catheter health indicators, and cycler alarms that may affect therapy outcomes.

  • Identify trends or concerns in therapy data and escalate findings to the PD program nurses according to established clinical pathways.

  • Support PD program nurses by documenting assessments, recommendations, and follow-up actions in accordance with privacy and clinical documentation standards.

  • Participate in data reviews and discussions with PD program nurses to ensure continuity of care and appropriate clinical response.

Virtual Care Coordination

  • Serve as a clinical liaison within PD Telecare, maintaining consistent communication with PD program nurses to coordinate data review, follow-up actions, and escalations.

  • Ensure all patient information, alerts, and escalations within the PD Telecare system are accurately documented and actioned in a timely manner.

  • Conduct the initial communication call with the PD program nurses to establish parameters for communication, the cadence of check-ins, and expectations for how ongoing collaboration and escalation will occur.

  • Contribute to continuous improvement of Telecare workflows, escalation tools, and digital resources that enhance efficiency and patient safety.

Quality, Reporting, and Trend Analysis

  • Track and report key performance indicators, including treatment adherence rates, catheter-related complications, and alarm resolution metrics.

  • Analyze recurring trends and collaborate with PD program nurses to develop targeted improvement strategies.

  • Identify opportunities to optimize Telecare escalation protocols, ensuring timely and effective clinical follow-up by PD program nurses.

  • Conduct documentation and process audits to verify alignment with Canadian renal nursing standards and internal quality expectations.

Education and Professional Development

  • Develop and maintain PD Telecare training materials for internal staff and PD program nurses, focusing on adherence monitoring, alarm interpretation, and catheter-health reporting.

  • Support onboarding and mentorship of new Telecare team members in PD data review, escalation processes, and communication protocols.

  • Maintain current knowledge of Peritoneal Dialysis guidelines, ISPD standards, and Canadian home-dialysis practices relevant to Telecare operations.

Collaboration and System Support

  • Collaborate closely with PD program nurses to ensure Telecare data and alerts are reviewed and acted upon in a timely, consistent manner.

  • Maintain and update internal Telecare resources, SOPs, SharePoint tools, and clinical support templates.

  • Contribute to cross-functional initiatives, including quality improvement projects, regional Telecare expansions, and Telecare analytics reporting.

What you'll bring:

  • Bachelor of Science in Nursing (BScN) or equivalent

  • Preferred 2-4 years of experience in Telehealth Nursing, Peritoneal Dialysis or Home Dialysis.

  • Registered Nurse (RN) in good standing with a Canadian provincial regulatory body (e.g., CNO, BCCNM, OIIQ).

  • Bilingual (English and French).

  • Demonstrated knowledge of APD therapy and cycler troubleshooting

  • Experience with Remote Patient Management systems, telehealth platforms, or virtual-care coordination.

  • Strong analytical and critical-thinking skills with the ability to interpret patient data and identify clinical priorities.

The estimated base salary for this position is $85,000 to $110,000 annually. The estimated range is meant to reflect an anticipated salary range for the position.   • Baccalaureat en sciences infirmieres (B.Sc.Inf.) ou equivalent

  • 2 a 4 ans d'experience en dialyse peritoneale, dialyse a domicile ou soins infirmiers en telesante
