• Conduct detailed psychosocial assessments to evaluate individual patient needs, encompassing emotional, social, and financial aspects.
• Deliver individual and group therapy sessions aimed at aiding clients in coping with issues such as grief, adjustment, and mental health challenges.
• Work with medical teams to formulate and implement effective treatment plans that address both the psychological and physical health of patients.
• Coordinate post-discharge services in collaboration with community organizations, ensuring seamless transitions back to everyday life.
• Provide education and emotional support to patients and their families regarding treatment options, resources, and next steps post-detox.
• Participate actively in professional committees, offering expertise on social work practices and advocating for patient needs within the institution.
• Implement crisis intervention strategies when necessary, ensuring immediate support for patients and their families during high-stress situations.
• Maintain meticulous documentation of all patient interactions and treatment plans in accordance with legal requirements and organizational policies.

The ideal candidate will be working primarily with individuals undergoing detoxification from substances, including alcohol and drugs. This population often faces numerous psychosocial challenges, including trauma, addiction, and co-occurring mental health disorders. You will have a direct impact on their emotional stability and overall treatment experience, guiding them through some of the most challenging times in their lives.

• Master's degree in Social Work from an accredited institution.
• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated understanding of mental health conditions, addiction treatment principles, and crisis intervention methodologies.
• Exceptional communication and interpersonal skills, with the ability to build rapport and trust with patients and their families.

• A minimum of two years of experience working in a healthcare or hospital setting is preferred, particularly in behavioral health or addiction services.
• Familiarity with electronic health record systems will be advantageous, allowing smooth documentation and data management.
• Experience engaging with community resources for assessing and supporting post-treatment needs.
