• Support primary care providers in recognizing and managing mental health and psychosocial issues, becoming an integral part of the multidisciplinary primary care team.
• Conduct comprehensive patient intakes with a focus on diagnostic and functional evaluations, making informed recommendations regarding treatment goals and plans.
• Provide brief, focused interventions for patients seeking mental health support, tailoring approaches to individual needs and circumstances.
• Advise primary care providers on the appropriate channels for patients, determining if they require specialty mental health services or can be managed within the primary care framework.
• Initiate follow-ups to monitor patient progress and evaluate the effectiveness of treatment approaches, ensuring timely adjustments as needed.
• Develop relapse prevention strategies for identified patients, assisting them in maintaining stable functioning in their everyday lives.
• Identify patients at risk for deteriorating conditions and help devise comprehensive prevention plans.
• Coordinate health service delivery related to behavioral health, liaising with other treatment providers, and securing patient consent for external referrals when necessary.
• Document patient progress, interactions, and relevant diagnostic information meticulously, ensuring comprehensive and compliant treatment charts.
• Keep primary care providers informed of patient developments and collaborate to formulate effective treatment plans.
• Promote healthy lifestyle changes and assist patients in self-management strategies that enhance their recovery and overall well-being.
• Contribute insights regarding behavioral services to clinic management and team members, continuously evaluating outcomes and program effectiveness.
• Support patients in adhering to prescribed medical treatments and coping strategies, managing any medication-related challenges.
• Uphold the Code of Ethical Conduct for Clinical Professionals, ensuring professional integrity and excellence in all interactions.
• Carry out additional duties as assigned to support the clinic's mission and operational needs.

Your work will primarily involve a diverse patient population, including individuals experiencing acute and chronic mental health issues, as well as those navigating psychosocial challenges. You will foster a supportive, empathetic environment that encourages active participation in their treatment and recovery processes, rooted in respect and sensitivity to individual backgrounds and experiences.

• A minimum of a Master's Degree in Clinical Counseling from an accredited institution.
• Active LPC (Licensed Professional Counselor) license in the State of New Jersey.
• Demonstrated knowledge in mental health assessment, treatment planning, and evidence-based therapeutic techniques.
• Thorough understanding of DSM-IV diagnoses, developmental psychopathology, and psychotherapeutic methodologies.
• Capable of conducting thorough evaluations and deriving individualized treatment plans.
• Strong interpersonal skills; ability to establish cooperative relationships with patients, families, and interdisciplinary team members.
• Competence in maintaining clinical records and healthcare documentation with a meticulous approach.
• Excellent communication skills, both verbal and written, with the ability to convey ideas effectively in a clinical setting.

• Bilingual proficiency in English and Spanish is highly preferred, enabling better communication with a broader range of patients.
• Experience in a community health setting or working within Federally Qualified Health Centers.

The role offers a competitive salary range between USD 87,000 and 105,000 per year, commensurate with experience and qualifications.
