Responsibilities

The Health Information Management Specialist (HIMS) is responsible for a timely and appropriate provision of patient health information to internal and external parties, which includes and is not limited to symptoms, diagnoses, medical histories, test results and procedures on behalf of HarmonyCares: Medical Group, Home Health and Hospice. The HIM Specialist complies with the Health Information Portability and Accountability Act (HIPAA), as well as state and federal regulations related to patient health information. This position processes external and internal medical records requests and fulfillments and maintains the record system for all incoming and outgoing patient records. 

Essential Duties and Responsibilities

  • Follows all HIM policies and procedures
  • Follows the standard process for reviewing, obtaining necessary approvals and fulfilling release of associated information
  • Follows the standard process for requesting HCMG patient records from external parties
  • Follows standard process for reviewing, separating, and uploading of requested patient records
  • Answer telephone and assist calls with process and questions related to external medical record requests
  • Monitor fax folder to process external medical record requests and fulfillments as needed
  • Monitor email and mail to process external medical requests and fulfillments as needed
  • Collaborate as needed with Compliance/Privacy, IT Security, Revenue Cycle, Coding, Clinical Teams and Legal to ensure streamlined policies and procedures
  • Maintains record system for all outgoing patient records
  • Assist Manager - Health Information Management with timely medical record submissions in response to payor audit requests
  • Supports mission, vision and core values which include but not limited to delivering compassionate support, attention and assistance to patients, families and members of our team 
  • Performs other duties as assigned
Qualifications

REQUIRED Knowledge, Skills and Experience

  • High School diploma or equivalent
  • Two (2) years' experience in a hospital or medical office
  • HIPAA knowledge training
  • Electronic Health Record (EHR) experience
  • Previous experience in the handling of patient health information, medical records document imaging and/or medical records
  • Ability to stoop, bend, push or lift up to 30lbs

PREFERRED Knowledge, Skills and Experience

  • Successful completion of Medical Terminology class
  • Registered Health Information Associate (RHIA) or Registered Health Information Technician (RHIT)
  • Aprima (HER) experience
  • Knowledge of CMS, state and Joint Commission regulations
