Job description

Purpose of Position- Provides Case Management to an assigned case load to include all the duties and responsibilities of an Intake or Ongoing Case Manager.  Mentors and provides coaching support for new hires and/or case managers needing remedial assistance or support. Coverage of contract-related meetings and special tasks or projects as assigned by Case management Supervisors or agency leadership.   

Essential Duties/Responsibilities 

  • Provides case management for assigned caseload to include all the duties and responsibilities of case management with targeted assistance for case coverage and/or high intensity cases.  

  • Supports the success of new hires and/or struggling case managers by:  

  • Providing shadowing opportunities for case management tasks to CMs. 

  • Consulting and counseling CMs on high intensity cases.  

  • Answering questions when a CM supervisor is not available.  

  • Shadowing and providing feedback to new hires/struggling CMs on CM tasks. 

  • Coaching new hires/struggling CMs on time management and triaging duties.  

  • Reviewing case management work of new hires and/or struggling CMs, providing feedback and updating CM supervisors on progress and/or areas of concern.  

  • Attends assigned meetings with contract officials, community partners and clients, representatives and/or advocates as assigned by CM Supervisor.  Examples include, but are not limited to, CCHA, PASRR, Team meetings and Service Plan meetings.  

  • Manages and/or assists with key projects/tasks as assigned by CM Supervisor to likely include, but not be limited to, CDASS annual revisions, ACF annual revisions, management of CDASS overspending reports, maintaining internal provider list, sending and reporting on RFP's, Long Term Home Health, HRC related issues and Investigations.  

  • Carries after-hours phone and responds to phone calls on a rotating basis 

  • Ensures all required documentation for ongoing funding and benefits is in accordance with applicable Federal and State regulations and A&I Avenues policies and procedures.  

  • Completion of timely, accurate, professional documentation within established timelines. 

  • This position requires regular use of a personal vehicle with liability insurance that meets A&I Avenue standards. 

  • This position requires regular use of a personal mobile device such as a smartphone or tablet.  

Other Duties/Responsibilities 

  • If position occupant has M.S.W., may supervise or assist in supervision of social work interns. 

  • Other duties as assigned.  

Job Qualifications 

Knowledge, Skill, and Ability: 

  • Solid skills in working with people with disabilities. 

  • Independent judgment and discretion in monitoring, advising, and advocating for clients and their families. 

  • Person-centered, creative problem solving and strengths-based solution-finding. 

  • Ability to manage workload independently and prioritize tasks/goals to meet timelines. 

  • Ability to work effectively in a team setting.  

  • Effective and professional written and verbal communication skills with clients and groups at all professional levels.  

  • Strong planning, time management, and organizational skills.  

  • Ability to work efficiently with various technology including word processing software, e-mail, databases, and general internet skills.  

  • Possession of a valid driver's license and ability to meet A&I Avenues driving requirements. 

  • Minimum of 1 year experience working in LTSS required 

Education: 

  • High School diploma required. 

  • Bachelor's Degree in human services related field is highly preferred.  

OR  

  • Five years' work/volunteer experience with long-term care and/or disabilities programs and services.   

OR 

  • Some combination of relevant education and experience appropriate to the requirements of the position.  

Working Environment/Physical Activities 

  • Effective with shifting roles, responsibilities, and expectations in a highly stressful and changing work environment.   

  • In the field workdays required regularly.  Living arrangements on the Colorado front range strongly preferred.  

  • Ability to work autonomously while remote.
