• Conduct thorough clinical assessments to evaluate patients' emotional and mental health needs.
• Develop personalized treatment plans that address the unique challenges faced by patients and their families in hospice care.
• Implement evidence-based therapeutic interventions such as solution-focused therapy and cognitive behavioral therapy to facilitate coping and adjustment.
• Document all clinical interactions accurately and promptly, maintaining up-to-date patient records in compliance with regulations.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to support comprehensive patient care.
• Provide educational resources and emotional support to families, helping them navigate the complexities associated with grief and loss.
• Stay abreast of current research and advancements in hospice and mental health care to refine practice and improve patient outcomes.

The LCSW will primarily work with patients facing terminal illnesses and their families, addressing complex emotional challenges during a sensitive and pivotal time in their lives. This role requires a compassionate and respectful approach towards individuals from diverse backgrounds, acknowledging their unique experiences and emotional responses to loss.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid LCSW licensure in the state of Michigan.
• Strong knowledge of solution-based psychotherapy methods and palliative care practices.
• Exceptional communication skills, both verbal and written, with a strong ability to engage effectively with patients and families.
• Demonstrated problem-solving skills and the ability to work collaboratively within a healthcare team.

• A minimum of 2 years of clinical experience, preferably in hospice or similar settings.
• Experience with mental health assessment tools and treatment planning specific to end-of-life care.
• Familiarity with electronic health record (EHR) systems to document and manage patient care efficiently.
