Key Responsibilities
  • Conduct comprehensive and ongoing assessments of clients’ mental health needs, ensuring that individualized treatment plans are developed and modified as necessary.
  • Effective communication is crucial; you will regularly collaborate with supervisors, direct care staff, family members, and other stakeholders to discuss patient progress and treatment changes.
  • Prepare tentative schedules detailing appointments, home visits, and monitoring activities to ensure efficient workflow and effective tracking of patient outcomes.
  • Coordinate essential home visits, performing in-home observations, chart reviews, and analyzing case-related data to enhance therapeutic interventions.
  • Implement program plans effectively and monitor therapy sessions to ensure adherence to best practices in client care.
  • Your participation in Interdisciplinary Team Meetings will be critical, allowing for comprehensive care discussions and strategic planning.
  • Document and complete detailed monthly and quarterly progress notes that reflect client advancements and areas needing attention.
  • Organize training sessions to certify team members on the implementation of treatment plans and best documentation practices.
  • Provide follow-up support through interviews and additional in-home visits to ensure clients continue to meet their goals.
  • Maintain meticulous records that facilitate easy access to information for various stakeholders, enhancing the continuity of care.
  • Act as a liaison for the public and mental health community, supporting the development of community-oriented goals and mental health outreach initiatives.
  • Assist in creating and refining procedural documentation and training materials that are essential for improving service offerings.
Patient Population / Client Focus

Your role will focus on a wide variety of client profiles, including both adults and youth, allowing for a rich and diverse range of therapeutic experiences. You will work with individuals facing challenges such as depression, anxiety, trauma, substance misuse, and other mental health issues. This diversity equips our counselors with the opportunity to make a tangible impact in different community demographics.

Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related mental health field is required.
  • Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Georgia is a must.
  • At least two years of experience in a counseling capacity, particularly within a clinical setting, is necessary to ensure effective patient care.
  • A background or specialization in areas such as family therapy, couples therapy, trauma recovery, or substance abuse counseling is highly desirable.
  • Strong understanding of legal, ethical, and professional standards within the mental health arena.
  • Exceptional interpersonal and communication skills to establish rapport with a wide range of clients.
  • Demonstrated ability to work collaboratively within healthcare teams and liaise with other professionals effectively.
Preferred Experience
  • Familiarity and proficiency with electronic medical record (EMR) software and other relevant technologies to streamline documentation and communication.
  • Prior experience in teletherapy or remote counseling practices is advantageous due to this being a primarily remote position.
