Job Summary and Responsibilities
As our Virtually Integrated Care RN, you will be a pivotal provider, empowering patients toward optimal health through comprehensive remote support and collaborative care coordination. You will be instrumental in bridging care gaps and enhancing engagement in a dynamic virtual environment.
Every day you will meticulously assess patient needs, develop personalized care plans, and provide direct clinical coaching. You will proactively monitor progress, coordinate diverse services, and leverage telehealth, diligently tracking compliance, documenting interactions, and contributing to quality improvement.
To be successful in this vital role, you will leverage advanced clinical expertise, communication, and technological proficiency for high-quality virtual care. You will collaborate seamlessly with providers and teams, optimizing complex pathways.
  • Collaborates virtually with physicians, nurses and other health care team members in the provision of clinical care for the patient. Assesses virtually the medical, physical, and psychological status of patients by reviewing patient records, obtaining a comprehensive problem-oriented health history and monitoring and evaluating laboratory, x-ray, EKG, and other diagnostic studies as medically indicated.
  • Interprets available data in order to establish a primary plan of care. The plan is developed through an interdisciplinary team approach and through multi-dimensional patient care conference, ongoing patient assessments, and re-evaluation of patient problems or needs. Prioritizes clinical information accurately and in a timely manner.
  • Responds to routine, urgent and emergent patient problems appropriately by demonstrating effective decision-making and provides follow-up to ensure that patient problems are resolved. Facilitates a collaborative spirit assuring teamwork by providing direction to staff. Monitors and provides feedback on performance of employees as requested by Nurse Director.
  • Works to prevent patient problems from occurring by: direct observation via telemedicine technology, proactive surveillance of adverse physiological trends, and assurance of appropriate prophylactic strategies, medical chart review, and appropriate reaction to patient alerts. Risk reduction will be done through participation in root cause analysis (RCA's), quality surveillance, as well as, compiling and interpreting large data sets of patient information.
  • Effectively communicates with members of the bedside interdisciplinary team in a collegial manner to provide safe and error-free care during routine and emergent conditions. Provides courteous and enthusiastic phone communication to employees at hospital sites. Coaches and mentors nurses and nursing support on the floor around the care of the patient, and set expectations as needed. Virtually assesses and reassesses patients as appropriate.
  • Advocate on behalf of patients and their families in regards to their cultural, spiritual and care needs. Virtually educates the patient and families on disease process, procedures and self-care to maximize the level of function and wellness upon discharge.
