This role is fully remote, providing flexibility to manage your work schedule while ensuring that we deliver timely and effective care to our patients. Our team utilizes virtual communication platforms and electronic health record (EHR) systems to maintain seamless collaboration and patient care oversight. You will receive comprehensive support from management and access to continual training and resources to enhance your clinical skills and knowledge.

As a Licensed Clinical Social Worker at Active Health & Hospice, your responsibilities will encompass:

• Conducting thorough mental health assessments, including psychosocial evaluations and risk assessments, to gain insights into each patient's unique needs.
• Collaborating with patients to develop and implement individualized treatment plans that reflect their specific goals and cultural backgrounds.
• Providing evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in individual, couple, family, and group settings.
• Offering crisis intervention services, effectively responding to acute patient distress and ensuring continuity of care through collaboration with healthcare professionals.
• Maintaining precise clinical documentation, including progress notes and treatment plans, in compliance with organizational policies and regulatory standards.
• Educating patients and their families about mental health issues, equipping them with practical strategies and knowledge to navigate challenges.
• Coordinating with a multidisciplinary team of psychiatrists, psychologists, nurses, and case managers to deliver comprehensive patient care and ensure optimal outcomes.
• Performing regular treatment outcome assessments to measure effectiveness and refine care strategies as needed.
• Keeping up-to-date with current research and best practices in mental health, actively integrating this knowledge into your clinical work.
• Adhering strictly to ethical guidelines, maintaining confidentiality and safeguarding patient information.

Your work will focus on patients receiving hospice care, many of whom face complex emotional and psychological concerns as they navigate the end-of-life journey. You will be tasked with providing tailored support for both the patients and their families, addressing issues related to grief, loss, and coping strategies, as well as fostering resilience and peace during this sensitive period.

The ideal candidate will possess:

• A Master’s degree in Social Work (MSW) from an accredited institution.
• An active state license as a Licensed Clinical Social Worker (LCSW).
• The ability to work autonomously while demonstrating strong collaborative skills within a team-oriented environment.
• Exceptional interpersonal and communication skills, enabling you to build rapport with individuals from diverse cultural and socioeconomic backgrounds.

Preferred candidates will have experience in hospice or palliative care settings, understanding the unique challenges this population faces. Familiarity with grief counseling techniques and end-of-life issues will be beneficial in optimizing care delivery. Additional certifications related to clinical therapy modalities and experience in multidisciplinary team settings will be advantageous.
