Your primary duties will include:

• Conducting thorough psychosocial assessments for individuals, couples, and families to identify mental health needs and determine the appropriate therapeutic interventions.
• Developing and executing individualized treatment plans using evidence-based practices, such as Cognitive Behavioral Therapy (CBT) and other clinical modalities.
• Collaborating closely with interdisciplinary teams, communicating effectively through documentation and meetings to ensure a cohesive approach to patient care.
• Providing case management services through consultation with hospital personnel and community resources, facilitating seamless access to care.
• Completing all required documentation ethically and efficiently to maintain high standards of accountability and compliance.
• Engaging in ongoing professional development and training opportunities to enhance therapeutic skills and stay current with best practices.

You will be working closely with patients who are experiencing a wide range of mental health challenges, including depression, anxiety, grief, and adjustment disorders, particularly in the context of hospice care. Our patients often require a sensitive and empathetic approach that addresses their emotional and psychological needs while considering the complexities of end-of-life issues.

To be considered for this role, candidates must possess:

• A current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
• A minimum of 2 years of clinical experience working in a relevant mental health field.
• A Bachelor’s degree in Social Work, Counseling, Psychology, or a related field.

While all qualified candidates are encouraged to apply, preference will be given to those who have:

• Experience in hospice or palliative care environments.
• Proficiency in conducting telehealth sessions and using related technologies.
• A background in crisis intervention and trauma-informed care.
