Key Responsibilities
  • Conduct comprehensive psychosocial assessments to understand clients' mental health concerns, strengths, and resources, using validated assessment tools.
  • Provide therapy to clients through individual, family, and group sessions utilizing evidence-based practices such as cognitive-behavioral therapy (CBT) and trauma-focused therapy.
  • Develop and implement individualized treatment plans that align with clientsโ€™ personal goals, ensuring open communication and collaboration with other healthcare professionals, including physicians and multidisciplinary teams.
  • Facilitate group therapy sessions, psychoeducation classes, and support groups to address various emotional and psychological needs within a community-focused setting.
  • Conduct crisis intervention services, providing timely risk assessments and developing safety plans for clients experiencing acute psychological distress.
  • Maintain accurate and thorough documentation of client interactions and treatment progress in compliance with ethical and legal standards.
  • Engage in mentorship and supervise less experienced staff when appropriate, fostering an environment of professional growth and development.
Patient Population / Client Focus

The primary client population for this role includes individuals receiving hospice care and their families, often dealing with grief, anxiety, and depression related to terminal illness. Your work will not only address their mental health needs but also support their emotional journeys, enhancing their overall quality of life during this sensitive period.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program is required.
  • Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee.
  • A minimum of two years of experience in a mental health or outpatient setting, particularly within hospice or palliative care is preferred.
  • Familiarity with evidence-based treatment modalities and approaches tailored to trauma and grief counseling.
  • Strong interpersonal, communication, and assessment skills to effectively engage clients and collaborate with interdisciplinary teams.
Preferred Experience

While a minimum of two years of clinical experience is required, candidates with more extensive backgrounds in grief counseling, team-based care, and hospice services will be highly regarded. Experience conducting family therapy and working in collaborative care environments is also beneficial.

Compensation and Benefits
