Key Responsibilities- Conduct thorough clinical assessments to develop tailored treatment plans that address emotional, psychological, and social challenges faced by patients and their support systems.
- Utilize evidence-based interventions, including solution-focused therapy, to assist patients in understanding and coping with their circumstances.
- Maintain detailed, accurate, and timely records of all clinical interactions, ensuring compliance with all relevant regulations and best practices.
- Collaborate with a dedicated multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to coordinate comprehensive care approaches.
- Educate patients and families about the hospice process, available resources, and coping strategies to foster emotional resilience during challenging times.
- Engage in ongoing professional development by staying updated on the latest research and treatment methodologies in mental health and hospice care.
In this role, you will work closely with terminally ill patients and their families. You will help them navigate the complex emotional landscape associated with end-of-life care, facilitating discussions about goals, wishes, and the grief process. Your contributions will play a vital role in enhancing the quality of life for patients and providing critical support to their families in coping with loss.
Qualifications- A Master’s degree in Social Work from an accredited institution is required.
- A valid LCSW license in the state of Georgia is mandatory to practice.
- Strong understanding of solution-focused psychotherapy interventions, particularly in the context of end-of-life care.
- Exceptional communication skills that foster rapport and trust with patients and their families, ensuring effective therapeutic relationships.
