Job description

The Clinical Pharmacist, Pharmacy Outreach (internally: Pharmacist, Pharmacy Outreach) is responsible for optimizing patient outcomes through telephonic interventions. This role focuses on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization.
 
Candidates should be comfortable working remotely from anywhere within the US, across all US time zones.
 
 
Please note: Valid pharmacist license in at least 2 of the following markets and a willingness to acquire more states as needed:    AL, KY, LA, MS, NC, KS, PA, UT, VA, WV, MI, DE
 
 
 
 
 
Primary Duties:
Clinical Execution & Patient Care
  • Counsel patients via telephonic outreach on medication adherence, address any medication related problems such as adherence barriers, polypharmacy, or high risk medication use, and make appropriate evidence-based recommendations to patients and Practice Care Teams
  • Perform focused clinical chart reviews as needed to inform patient interventions
  • Obtain and document patient commitment to medication adherence and address barriers in real time
  • Escalate complex clinical cases to prescribers or interdisciplinary teams as appropriate
  • Meet and sustain performance goals (e.g., STARs, HEDIS) 
Clinical Innovation and Resource Optimization
  • Collaborate with Physicians, clinical and administrative staff to optimize the Medication Management workflow and patient experience
  • Proactively identify gaps in medication management through patient interactions and translate patient insights into targeted recommendations that drive improvements in practice operations and clinical workflows
  • Participate in the design, implementation, and evaluation of pilot programs or process changes ("test and learn") within the outreach model
  • Serve as a clinical subject matter expert, leveraging clinical pharmacy expertise to support organizational objectives and adapt to evolving healthcare needs
  • Develop and maintain standardized resources, educational tools, and clinical protocols to support improved medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patients
  • Other duties as assigned 
Minimum Qualifications:
  • Doctor of Pharmacy (PharmD)
  • Active, unrestricted pharmacist license in the state of practice; Valid multi-state licensure in at least two of the following states: AL, KY, LA, MS, NC, KS, PA, UT, VA, WV, MI, DE
  • Willingness to obtain and maintain additional licenses within first 60 days of hire
  • Minimum of 3 years experience in value-based care, ambulatory care, or community pharmacy settings
  • In depth knowledge of pharmacotherapy and pharmacy practice
  • Strong verbal and written communication skills; Ability to translate complex clinical data into clear, understandable language for patients and care teams
  • Proficiency in telephony platforms and various Electronic Health Record (EHR) systems
  • Ability to work in a fast paced environment and adapt quickly to change
  • Ability to work between 8 AM - 7 PM EST as dictated by business needs
Preferred KSA's:
  • Familiarity with Medicare, HEDIS, and STAR ratings  
Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer
$115,000 - $135,000 a year
Salary Range: $115,000 to $135,000 a year + bonus + equity
Compensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience.
