• Conduct thorough psychosocial assessments using interviews, observations, and detailed reviews of medical records to identify emotional, social, and environmental factors that may affect patient recovery.
• Provide empathetic counseling and support to patients and their families, focusing on adjustments to new lifestyles and addressing psychosocial issues that may arise during rehabilitation.
• Collaborate closely with the rehabilitation team to create and implement individualized treatment plans that include psychosocial considerations, ensuring holistic care that focuses on both mental and physical health.
• Assist patients in navigating and accessing community resources, such as support groups, vocational rehabilitation services, and housing assistance, tailored to their unique needs.
• Educate patients and their families about available community resources and services that facilitate their continued recovery and well-being post-rehabilitation.
• Engage actively in discharge planning, ensuring a seamless transition for patients back to their homes or other necessary care environments.

Our patients are diverse, ranging from those recovering from strokes and spinal cord injuries to individuals who have undergone orthopedic surgeries and face challenges associated with neurological disorders. As an LCSW, you will directly impact our patients' emotional and social resilience, guiding them through their recovery phases and helping them develop coping strategies for ongoing challenges.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• Demonstrated understanding of psychosocial factors influencing recovery, including adjustment issues, coping mechanisms, and grief.
• Excellent communication and interpersonal skills, with the ability to establish rapport and trust with patients and their families.

• Previous experience in a rehabilitation or medical setting is highly preferred, enabling you to understand the complexities of patient care in such environments.
• Familiarity with the challenges faced by individuals recovering from mobility impairments, as well as knowledge of the resources available to support them, will enhance your effectiveness in this role.
