• Conduct comprehensive psychosocial assessments through client interviews, observations, and review of medical records to evaluate the emotional, social, and environmental factors affecting patients’ recovery.
• Provide individual and family counseling to assist patients in addressing challenges related to their adjustment and emotional well-being.
• Collaborate with a diverse rehabilitation team to develop personalized treatment plans that incorporate patients' psychosocial needs, ensuring a holistic approach to care.
• Assist patients in navigating community resources, such as support groups, housing assistance, and vocational rehabilitation services to promote their independence.
• Educate patients and their families on available support services post-rehabilitation to facilitate successful integration back into their communities.
• Participate actively in discharge planning, coordinating transitions to home or other care settings while ensuring continuity of care.

You will work primarily with individuals experiencing a range of behavioral health and substance use issues in a detox setting. The work is emotionally intensive, requiring a deep understanding of the unique challenges faced by these patients, such as trauma, addiction, and chronic health conditions. Your efforts will significantly impact their recovery journey by providing crucial emotional support and resources that address their psychosocial barriers.

• A Master’s degree in Social Work (MSW) from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong understanding of mental health issues, trauma, and coping mechanisms, particularly in the context of rehabilitation and detoxification processes.
• Excellent communication and interpersonal skills, which are vital for engaging with patients and their families effectively.

• Prior experience in a rehabilitation or detoxification setting is highly preferred.
