This position requires a full-time commitment, adhering to standard business hours during the weekdays, with occasional evening and weekend shifts based on patient needs and your assigned caseload. • Conduct comprehensive psychosocial assessments, utilizing interviews, observational techniques, and the review of medical records to deeply understand the emotional, social, and environmental factors influencing each patient’s recovery journey.
• Provide empathetic counseling and support to patients and their families, focusing on adjustment issues and mental well-being to enhance overall recovery processes.
• Collaborate closely with the rehabilitation team to develop tailored treatment plans that integrate the psychosocial needs and personal goals of each patient.
• Facilitate access to community resources, such as support groups, vocational rehabilitation programs, or housing assistance, ensuring patients receive holistic support that extends beyond our facility.
• Involve family members in the rehabilitation process by providing education about available community resources and effective post-rehabilitation services.
• Partake in discharge planning to prepare patients for transitions to home or other care settings, ensuring that all aspects of their care are addressed comprehensively.

As a Licensed Clinical Social Worker at Bridge Way Reflections Therapy, you will be working primarily with a diverse patient population that includes individuals recovering from various conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your role is vital in addressing not only their emotional needs but also facilitating their rehabilitation journey, ultimately supporting their reintegration into the community.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of adjustment issues, grief counseling, and various coping mechanisms essential for guiding patients through challenging transitions.
• Exceptional communication and interpersonal skills to effectively collaborate with multidisciplinary teams and interact with patients and their families.
