Job Details

The Ambulatory Clinical Pharmacist will service outpatient specialty clinics. The pharmacist, through telephonic communication, will support patients filling with Reading Hospital Pharmacy and seen at a partnering outpatient clinic. Support may include performing follow-up evaluations, counseling on medications, assessing for any medication-related problems, and providing recommendations to prescriber. Additionally, the pharmacist will partner with other pharmacy team members to ensure access and affordability to medications. Documentation will be completed in patient's electronic medical record (EMR). A clinical pharmacy background with an understanding of retail pharmacy operations and strong communication skills is optimal to succeed in this role.

Location: Hybrid/remote (Pennsylvania) with some onsite expectations required in West Reading, PA for training, quarterly meetings, and support as needed

Hours: Full-time, Monday to Friday with no weekends or holidays typically required

Responsibilities:

  • Complete thorough medication reconciliation.

  • Identify medication-related issues.

  • Communicate clinical recommendations to providers.

  • Provide initial and follow-up medication counseling to patients.

  • Maintain patient schedule and arrange appropriate follow-ups to assess medication safety and efficacy.

  • Anticipate and address medication questions, such as formulary product selection, product dosing, ADR's, and other considerations.

  • Act as financial advocate for patient when needed, leverage charitable foundations, state programs, and/or manufacturer programs to ensure medication affordability.

  • Prepare and submit prior authorizations, manage denials/appeals.

  • Demonstrate ability to navigate various platforms and document appropriately in EMR.

  • Cross-train to ambulatory pharmacy for purposes of remote order entry and verification.

  • Perform other job duties as assigned.

Education:

Normally requires Bachelors of Science Degree or Pharm D degree from an accredited School of Pharmacy a Degree in Pharmacy from an accredited College of Pharmacy; current, unrestricted pharmacist license through the appropriate state board of pharmacy. Also requires broad training and/or experience in the fields of business administration, sales, marketing, information technology and retail/ambulatory pharmacy. Normally requires one (1) to three (3) years of directly related and progressively responsible retail/ambulatory care pharmacy experience.

What your background should look like:

  • PharmD degree required.

  • PGY1 residency or previous clinical experience is highly desirable.

  • Unrestricted pharmacist license in the state of PA is required. If out of state, must obtain licensure within 90 days of receiving offer letter.

  • Experience with 340B purchasing program is beneficial.

Skills:

  • Must be proficient in Microsoft Office, including Word, Excel, and PowerPoint.

  • Experience in Epic or similar EMR platforms.

  • Experience with retail pharmacy prescription processing and claims adjudication.

  • This role involves working with broad array of complex disease states and medical conditions; candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records.

  • Excellent communication skills, with the ability to communicate effectively both orally and in writing

  • Ability to work quickly and accurately under time and volume constraints

  • Strong leadership and interpersonal skills

  • Ability to resolve patient issues quickly and creatively in order to improve customer satisfaction

  • Strong organizational skills, attention to detail, and analytical skills
