You will work within an outpatient office framework, collaborating virtually with a team of dedicated healthcare professionals. As part of a multidisciplinary team, you will have the opportunity to engage in regular team meetings, case conferences, and collaborative care planning sessions aimed at optimizing patient outcomes.

• Conduct comprehensive mental health assessments that consider the interplay between psychiatric and medical conditions, ensuring a well-rounded understanding of each patient’s needs.
• Design and implement integrated treatment plans that address both mental health and physical health aspects, utilizing evidence-based methodologies tailored to individual circumstances.
• Facilitate individual and group therapy sessions that empower patients to confront their challenges, utilizing therapeutic techniques that promote psychological growth.
• Work collaboratively with physicians, nurses, and other medical staff members to coordinate seamless patient care and ensure appropriate treatment strategies are followed.
• Provide crisis intervention services, offering immediate support and guidance during mental health emergencies.
• Educate patients about the relationship between mental and physical health, equipping them with the skills necessary for managing their overall well-being.
• Maintain comprehensive records in compliance with legal and ethical standards, utilizing electronic health record (EHR) systems effectively to document patient progress.
• Engage actively in ongoing professional development and case review processes to enhance clinical skills and maintain current knowledge of best practices.

Your primary focus will be on individuals with complex mental health needs, particularly those facing co-occurring medical conditions such as autoimmune disorders, chronic pain syndromes, and substance use disorders. Our patients often require an integrative approach that acknowledges the interconnectedness of mental and physical health. Your role will be pivotal in guiding them through their unique challenges, fostering resilience, and facilitating recovery.

• Candidate must hold a Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Possess a current and valid license as an LMHC, LCSW, LCSW-R, Licensed Psychoanalyst, Psychologist, or LMFT in the state of New York.
• Demonstrated experience in dealing with the mental health needs of individuals experiencing co-occurring medical conditions.
• Excellent interpersonal and communication skills are essential to build rapport with patients and collaborate with multidisciplinary teams.
• A commitment to maintaining ethical standards and patient confidentiality in all interactions.
• Familiarity with EHR systems and various therapeutic interventions is preferred.

• Experience in integrated care settings, particularly with a focus on individuals with chronic and complex health issues.
• Skills in crisis intervention and management, with a proven track record of effective patient support during emergencies.
• Active involvement in team-based care and interdisciplinary planning would be advantageous.
