• Conduct comprehensive assessments and develop personalized treatment plans for clients, ensuring to address both pre-crisis and crisis interventions as necessary.
• Maintain accurate and timely documentation that meets regulatory standards and complies with best practices.
• Utilize evidence-based screening tools to assess clients' needs and symptoms, working together with the clinical team to implement individual care strategies.
• Collaborate with interdisciplinary teams to create and execute positive behavior support plans tailored to each client's unique situation.
• Actively engage in all licensing and accreditation processes to uphold the highest standards of care.
• Serve as a liaison to various service systems, state agencies, and community partners, ensuring effective communication and cooperation.
• Establish and maintain open lines of communication with all stakeholders, enhancing the agency's mission and promoting community awareness.
• Network with local, state, federal, and private organizations to promote programs, identify potential partnerships, and secure new funding sources.
• Commit to ongoing professional development by attending relevant conferences and training sessions, representing Grace Hospital where applicable.
• Promote and integrate the agency's mission, values, and principles into your daily practice.
• Perform other related duties as assigned by the Clinic Director to support team objectives.

• A Master’s Degree in a behavioral health field is a prerequisite for this position.
• Must possess an independent LMHC License issued by the state board.
• Proven ability to maintain clinical records, prepare reports, and document client interactions efficiently.
• Strong interpersonal and communication skills, with the ability to work collaboratively within an interdisciplinary team.
• Ability to articulate thoughts and ideas clearly and effectively, both verbally and in writing.
• Demonstrated capacity for exercising judgment and making independent decisions grounded in professional expertise.

• Previous experience in a telehealth setting is advantageous.
• Familiarity with evidence-based practices in mental health counseling.
• Experience working with diverse populations and knowledge of cultural competency principles.

The salary for this position ranges between $86,000 and $103,000 annually, commensurate with experience.
