• Conduct thorough assessments of patients and their families to understand their unique emotional, social, and psychological needs.
• Create and implement personalized treatment plans that address these needs, incorporating evidence-based therapeutic practices.

• Maintain accurate and confidential patient records in compliance with HIPAA regulations and organizational policies.

As an LCSW within our hospice center, you will work primarily with patients facing terminal illnesses and their families. Your expertise will assist them in coping with grief, loss, and the emotional challenges associated with end-of-life transitions. The diverse backgrounds of our patients require an adaptable and culturally sensitive approach to care that respect their individual values and preferences.

• A Master's degree in Social Work from an accredited institution.
• An active Licensed Clinical Social Worker (LCSW) license in the state of Arizona.
• Demonstrated expertise in providing therapeutic support to individuals experiencing trauma, anxiety, and depression.
• Exceptional interpersonal, communication, and problem-solving skills.
• A proven ability to work effectively within a collaborative healthcare team.

Experience in hospice, palliative care, or working with individuals facing chronic or terminal illnesses is highly desirable. Familiarity with solution-focused therapies and trauma-informed care will be advantageous in effectively supporting our patient population.
