• Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and environmental factors impacting their recovery process.
• Utilize evidence-based counseling techniques to provide individualized therapy to patients and their families, focusing on adjustment issues and emotional resilience.
• Collaborate closely with the interdisciplinary rehabilitation team to develop and implement tailored treatment plans that reflect each patient's unique psychosocial needs.
• Assist in connecting patients with appropriate community resources, such as vocational rehabilitation, housing assistance, and support groups.
• Participate actively in discharge planning to facilitate smooth transitions for patients returning to home or other care settings.
• Document clinical observations, treatment plans, and progress notes in compliance with healthcare regulations and internal monitoring systems.

The patients you will serve comprise a diverse demographic, including individuals recovering from substance abuse, neurological disorders, orthopedic surgeries, and trauma. Each patient presents unique emotional and social needs, and your role will be crucial in guiding them through significant adjustments and challenges during their rehabilitation journey.

• A Master’s degree in Social Work from an accredited program.
• Active professional licensure as a Licensed Clinical Social Worker (LCSW) within the state of Illinois.
• Strong understanding of adjustment disorders, grief counseling, and coping strategies tailored to a diverse patient population.
• Exceptional interpersonal and communication skills to build rapport with patients, families, and interdisciplinary team members.

• Previous experience in a rehabilitation, medical, or supportive care setting is highly desirable.
• Familiarity with community resource networks and programmatic options available for patients post-rehabilitation.
• Experience utilizing electronic health records (EHR) and clinical documentation software.
