• Conduct comprehensive psychosocial assessments by utilizing interviews, observations, and thorough reviews of medical records to evaluate the psychosocial aspects impacting patient recovery.
• Provide empathetic counseling and support to patients and their families, assisting them in addressing adjustment issues and emotional challenges that may arise during the rehabilitation process.
• Collaborate closely with the rehabilitation team to create and implement individualized treatment plans that reflect each patient’s unique psychosocial needs, preferences, and goals.
• Guide patients and their families in accessing essential community resources such as support groups, vocational rehabilitation services, or housing assistance to facilitate their recovery journey.
• Participate actively in discharge planning, ensuring smooth transitions for patients returning home or moving to other care settings, thereby enhancing their reintegration success.

Your work will directly impact a diverse patient population dealing with various conditions, including those recovering from stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. By addressing their emotional and social needs, you will play a pivotal role in their rehabilitation journey, promoting not only their independence but also their overall well-being within the community.

• A Master’s degree in Social Work from an accredited program is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• Strong knowledge of psychosocial issues related to rehabilitation, grief, and effective coping mechanisms.
• Excellent communication and interpersonal skills are needed to effectively support patients and collaborate with healthcare teams.

• Experience in a rehabilitation or medical setting is highly preferred and will be considered an asset.
