Key Responsibilities

  • Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, to enhance their post-rehabilitation integration.
  • Participate actively in discharge planning, ensuring seamless transitions and continuity of care as patients move towards home or alternative care environments.
Patient Population / Client Focus

Qualifications
  • A Master's degree in Social Work from an accredited institution.
