• Conduct comprehensive psychosocial assessments through interviews, observations, and medical record reviews to understand the emotional, social, and environmental factors affecting patient recovery.
• Provide empathetic counseling and emotional support to patients and their families, tackling adjustment problems, grief, and coping strategies.
• Work closely with the rehabilitation team to develop personalized treatment plans that holistically address patients' psychosocial needs and identify goals for their recovery.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, to enhance their post-rehabilitation integration.
• Participate actively in discharge planning, ensuring seamless transitions and continuity of care as patients move towards home or alternative care environments.

Our center focuses on a diverse patient population requiring detoxification support and rehabilitation services. You will engage directly with individuals overcoming addiction, as well as those facing psychosocial challenges due to underlying medical or emotional conditions. Your role will be pivotal in fostering their well-being and facilitating their journey back to functional independence within their community.

• A Master's degree in Social Work from an accredited institution.
• Current, active licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of psychosocial issues affecting rehabilitation and recovery, including crisis intervention techniques.
• Outstanding communication, active listening, and interpersonal skills to effectively engage with patients and their families.

• Previous experience in a rehabilitation or medical setting, preferably with a focus on detoxification services.
• Solid knowledge of community resource networks that support recovery and vocational rehabilitation.
• Experience with interdisciplinary team collaboration and treatment planning.
