Job description
Other duties may be assigned. 1. Assist in the identification and development of member health education needs through health assessment activities that may include online health risk appraisal, telephonic outreach, etc.
2. Provide telephonic outreach and counseling on member compliance with nationally accepted health prevention guidelines and general health and wellness strategies. 3.
Perform research and communicate on relevant topics in health promotion and disease prevention. 4. Facilitate collaboration between health education resources, the community and the company.
5. Comply with applicable regulatory and licensing requirements such as NCQA, CMS, URAC, etc. 6.
Monitor, evaluate and report programs' effectiveness (e.g. outcomes). 7.
Prepare presentation/display materials and conduct targeted education/wellness programs targeting specific health strategies for designated populations. 8. Serve as liaison between internal and external stakeholders to champion and develop programs aimed at prevention, wellness and disease management.
EDUCATION AND EXPERIENCE Relevant combination of education and experience may be considered in lieu of degree. EDUCATION 1. Bachelor's degree in health education, Health Promotion, Nutrition, Nursing, Exercise Physiology or Social Work required.
2. Master's degree in related discipline is preferred. 3.
Certified Health Education Specialist (CHES) certification, Licensed Registered Nurse (RN), American College of Sports Medicine (ACSM) certification. 4. Medical licenses require current unrestricted state of Michigan licensure.
5. Valid and unrestricted Michigan driver's license is required. 6.
Continuous learning, as defined by the Company's learning philosophy, is required. 7. Certification or progress toward certification is highly preferred and encouraged.
EXPERIENCE 1. Two (2) years of experience in a health promotion/education setting, counseling on health topics such as nutrition, exercise, cholesterol, hypertension, stress, alcohol and smoking. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Microsoft Office Suite) and proven administrative abilities. 6. Working knowledge and understanding of health behavior change theories, counseling, interviewing techniques, strategies and application required.
7. Ability to plan, implement and evaluate employee disease prevention and health promotion programs. 8.
Reliable transportation, current auto liability coverage and an acceptable driving record as travel is required. 9. Knowledge of NCQA, CMS, URAC, HEDIS standards as applicable to designated department assignment.
10. Other related skills and/or abilities may be required to perform this job based on designated 11. departmental assignment.
APPENDIX Diabetes Management Program ADDITIONAL SUMMARY Responsible for delivering a structured, evidence-based diabetes education and coaching program for members with diabetes. The role is focused on improving self-management, medication adherence, lifestyle modification and clinical outcomes for a diverse member population. ESSENTIAL DUTIES AND RESPONSIBILITIES 1.
Provide coaching for diabetes condition management including self-management, glucose monitoring, medication use, nutrition, physical activity, stress reduction, reducing complications, etc. 2. Use motivational interviewing and behavior change techniques to set individualized goals, develop action plans and track member progress.
3. Refer members to internal or external resources (programs, providers) and collaborate with those resources to ensure a seamless continuum of care. 4.
Document all interactions, assessments, care plans, etc. within required timeframes. 5.
Comply with applicable regulatory and licensing requirements such as NCQA and CMS. 6. Monitor, evaluate and report programs' effectiveness (e.g., outcomes) and participate in continuous improvement activities.
EDUCATION AND EXPERIENCE 1. Two (2) years of experience in delivering chronic condition coaching or diabetes education. 2.
