Key Responsibilities
  • Conduct thorough evaluations and assessments of children under care to identify developmental needs and establish individualized support plans.
  • Collaborate as part of a multidisciplinary team to create and implement Individualized Family Service Plans (IFSPs), ensuring that each child’s unique circumstances are addressed.
  • Monitor and assess the ongoing progress of children, offering adjustments to strategies as needed, and maintaining documentation compliant with federal and state regulations.
  • Provide hands-on support to families, delivering educational resources, skills training, and emotional guidance tailored to their child's developmental needs.
  • Engage effectively with children, families, and colleagues, employing a coaching paradigm to facilitate learning in natural environments.
  • Ensure the delivery of services adheres to established best practices, particularly focusing on Natural Learning Environment Practices.
  • Develop and maintain strong relationships with stakeholders, including other healthcare professionals, to foster a cohesive support network for families.
Patient Population / Client Focus

The population served primarily comprises infants and toddlers experiencing disabilities or developmental delays. This role is crucial in delivering early intervention services designed to enhance physical, cognitive, social/emotional, and communication skills. Qualifications
  • Master's Degree or Doctorate in Social Work from an accredited institution.
  • Current licensure as a Clinical Social Worker (LCSW) in the state of Georgia, as mandated by the Department of Public Health.
