As a remote position based in Mount Holly, this role allows for flexibility while maintaining an engaging and collaborative atmosphere with our multi-disciplinary team. Our hospice setting is designed to support both the emotional and clinical aspects of care, enabling you to carry out your duties effectively from the comfort of your home. You'll have access to various online tools and resources that facilitate communication and collaboration with your colleagues and clients.

• Conduct thorough clinical assessments of patients and their families, developing personalized care plans that reflect each patient’s unique situation, preferences, and values. You will utilize evidence-based practices to ensure the best outcomes for those in your care.
• Implement a range of therapeutic interventions including, but not limited to, cognitive-behavioral therapy (CBT) and grief counseling, to help clients manage their emotional responses and transitions during hospice care.
• Maintain accurate and up-to-date clinical documentation while adhering strictly to HIPAA regulations to ensure confidentiality and ethical handling of patient information.
• Collaborate with a diverse team of healthcare providers, including physicians, nurses, and chaplains, to create and apply a comprehensive care approach that addresses the physical, emotional, and spiritual needs of patients.
• Engage in ongoing case consultations and peer review sessions to enhance quality improvements and incorporate the latest evidence-based practices into care delivery.
• Educate patients and their families about coping strategies, available resources, and support services, equipping them to manage the emotional and practical challenges that accompany end-of-life care.
• Stay informed about developments in the social work field, actively seeking out professional development opportunities to continually refine your skills and knowledge.

In this role, you will predominantly work with patients facing terminal illnesses and their families. Your compassionate support will empower them to address complex emotional needs, find solace during their bereavement process, and navigate difficult conversations around death and dying. You will play a crucial role in fostering an environment of dignity and respect, impactful in their last moments.

• A Master's degree in Social Work from an accredited institution, demonstrating your commitment to excellence in the field.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in Virginia, confirming your professional competence and adherence to industry standards.
• Proficiency in therapeutic methodologies, particularly those relevant to hospice and palliative care settings, along with a deep understanding of psycho-social dynamics surrounding end-of-life issues.
• Strong communication and interpersonal skills are essential, allowing for the effective establishment of rapport and trust with clients and their families.
• Critical thinking capabilities that facilitate problem-solving in complex situations will be highly beneficial.

• Previous experience in hospice, palliative care, or similar settings is strongly preferred, providing you with a solid foundation for understanding the unique challenges faced by clients.
• Certification in grief counseling or trauma-informed care would be an additional asset, enabling you to offer specialized support to clients.

We offer an annual salary ranging from USD 80,000 to 100,000, commensurate with experience and qualifications.
