As a Family Model Provider, you play a vital role in creating a safe, supportive, and nurturing environment for individuals with intellectual and developmental disabilities, as well as those facing mental health or substance use challenges.

This opportunity is ideal for individuals or families who believe in connection, care, and community. As an Independent Contractor, you won’t be an employee of Hands In Unity and are not eligible for company benefits, but you will gain the reward of transforming lives including your own.

A Home-Centered Approach to Care

Do you have space in your home and a desire to support someone in living a fuller, more independent life? As a Family Model Provider, you welcome an individual into your home and become a steady, caring presence in their daily life. You help them grow, achieve personal goals, and feel like a valued member of a family.

Hands In Unity is dedicated to providing person-centered services across East Tennessee. Our Family Model program allows individuals to live in a natural home setting where they receive guidance, encouragement, and support tailored to their unique needs, preferences, and dreams.

As a Family Model Provider, you will:

  • Create a warm, family-like environment that promotes dignity, respect, and independence.
  • Support daily living activities such as meal preparation, personal care, and household routines.
  • Encourage participation in education, work, and community activities.
  • Provide transportation and help coordinate medical, therapeutic, and recreational appointments.
  • Foster social, emotional, and spiritual well-being.
  • Maintain a safe, clean, and welcoming home environment.
  • Follow and support the individual’s Person Centered Support Plan.
  • Collaborate with care teams and attend meetings to support progress and communication.

Hands In Unity is committed to your success and well-being. We provide:

  • Ongoing training, including crisis support and skill-building techniques.
  • Guidance from a dedicated support team.
  • Assistance with respite care when needed.
  • Regular supervision and check-ins tailored to your needs.

Compensation is based on the level of care required and is provided on a contractual basis.

To become a Family Model Provider, you must:

  • Be 18 years or older with a high school diploma or GED.
