Key Responsibilities

In your role as a Licensed Professional Counselor, you will:

• Conduct comprehensive patient intakes, focusing on both diagnostic assessments and functional evaluations, and make informed recommendations to primary care providers regarding treatment goals and plans.
• Provide brief, targeted interventions to support patients in navigating mental health challenges effectively.
• Assist the primary care team in identifying and managing mental health and psychosocial issues among patients, ensuring cohesive care across disciplines.
• Follow up with patients to monitor their progress and discuss the efficiency of treatment strategies, making adjustments as necessary.
• Develop relapse prevention plans, equipping patients with skills to maintain stability and address potential setbacks.
• Identify “at-risk” patients and collaborate on preventive strategies to mitigate the deterioration of their mental health.
• Integrate behavioral health services with medical care, promoting overall wellness through coordination with other treatment providers, while respecting patient confidentiality.
• Document comprehensive patient progress notes and maintain updated clinical records in compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.
• Provide ongoing consultations to clinic management regarding behavioral health needs and program evaluation metrics.
• Educate patients on self-management techniques and lifestyle modifications, empowering them to take an active role in their recovery.

Patient Population / Client Focus

You will engage with a diverse client base within our community, including individuals facing various mental health challenges and psychosocial issues. Our approach emphasizes cultural competence and sensitivity, allowing you to build meaningful connections with patients and their families. You will facilitate their journey toward improved mental health, collaborating closely with primary care providers and specialty mental health facilities when necessary.

Qualifications

The ideal candidate will possess:

• A Master’s Degree in Clinical Counseling or a related field.
• A current Licensed Professional Counselor (LPC) license.
• Comprehensive knowledge of mental health assessment practices, treatment planning, developmental psychopathology, and behavioral therapy techniques.
• Familiarity with DSM-IV diagnoses and psychotherapeutic methodologies.
• A solid understanding of the social effects of mental health conditions on individuals and families.
• The ability to establish rapport and collaborative relationships with patients, staff, and community resources.
• Strong documentation skills and the capacity to communicate clearly and effectively.

Preferred Experience

Preferred candidates will also have:

• Experience in an integrated healthcare setting, working alongside primary care teams.
• Bilingual proficiency in English and Spanish.
• Knowledge of community resources, ensuring patient referrals to appropriate services.

Compensation and Benefits

This full-time Licensed Professional Counselor position offers a competitive salary range of USD 87,000 – 105,000 annually. In addition, our organization provides:
