As a remote position, the Clinical Social Worker will have the flexibility to work from home while being integral to our local community health initiatives in Manteca, California. The role includes maintaining close communication with multidisciplinary teams, participating in virtual meetings, and providing services via telehealth platforms. • Conduct comprehensive bio-psychosocial assessments, addressing mental health, substance abuse, and medical needs to inform treatment strategies for clients across their lifespan.
• Deliver therapeutic services including individual and group counseling, crisis intervention, and ongoing support, adapting interventions to meet the unique needs of individuals, couples, and families.
• Document clinical evaluations, progress notes, and treatment plans using electronic health record (EHR) systems, ensuring accurate and timely communications with team members and healthcare providers.
• Facilitate access to local resources, including social work support and referrals for medication assistance programs while serving as a liaison between clients and community agencies.
• Engage with quality improvement initiatives to enhance patient care and outcomes through data collection, analysis, and feedback processes.

The Clinical Social Worker will work with a heterogeneous client base that includes individuals from various cultural backgrounds, ages, and socioeconomic statuses seeking support for mental health issues, substance use disorders, and psychosocial challenges. You will play a pivotal role in creating a safe space for each client, where they can share their experiences and find pathways toward healing and recovery.

• A current state license as a Licensed Clinical Social Worker (LCSW) is mandatory, ensuring adherence to professional standards and ethical practices.
• A Master’s degree in Social Work from an accredited institution is required, along with a minimum of three (3) years of post-licensure experience providing direct clinical services.
