• Conduct thorough clinical assessments via telehealth sessions to determine patient needs and strengths.
• Develop tailored treatment plans incorporating evidence-based therapeutic interventions appropriate for hospice care environments.
• Utilize therapeutic modalities including cognitive-behavioral therapy (CBT) and mindfulness techniques tailored to individual patient situations.
• Maintain detailed clinical records and ensure documentation is completed in a timely manner, utilizing our electronic health record (EHR) system.
• Collaborate with a diverse team of healthcare professionals, including physicians, nurses, and chaplains, to coordinate care that aligns with patient goals.
• Provide emotional education and support to patients, families, and caregivers navigating end-of-life care decisions.
• Stay up-to-date with the latest research and developments in psychotherapy and hospice care to enhance service delivery.

As a Senior LCSW, you will work primarily with patients in hospice care—individuals facing serious and terminal illnesses. Your role involves supporting not only the patients but also their families, guiding them through complex emotional landscapes, offering coping strategies, and facilitating difficult conversations about end-of-life wishes and bereavement processes.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Connecticut.
• Demonstrated understanding of solution-based and grief-related therapeutic interventions.
• Exceptional communication, interpersonal, and problem-solving skills.

• Prior experience in hospice, palliative care, or similar settings is highly desirable.
• Familiarity with healthcare documentation systems, preferably electronic health records.
• Experience in teaching or mentoring other staff members or interns.

This full-time position offers a competitive salary ranging from USD 80,000 to 100,000 per year, commensurate with experience. In addition to salary, our benefits program includes:
