Your role will primarily involve engaging with patients virtually and coordinating their care from a distance, emphasizing comfort and accessibility for families facing challenging circumstances. • Conduct thorough psychosocial assessments to identify and address patients' emotional, social, and financial needs.
• Deliver individual and group counseling sessions, focusing on adjustment issues and coping strategies for grief and loss.
• Collaborate closely with interdisciplinary medical teams to create holistic care plans that address the multifaceted needs of our patients.
• Develop comprehensive discharge plans and work with community organizations to facilitate post-discharge services.
• Provide crisis intervention and emotional support to patients and their families during critical times.
• Engage with hospital committees to provide expertise in social work-related matters and enhance service delivery.
• Ensure timely and accurate documentation in compliance with organizational, legal, and ethical standards.

Your primary focus will be on patients in the Hospice Center, where you will support individuals dealing with terminal illnesses. This role requires a deep understanding of the emotional and psychological impacts of terminal diagnoses on patients and their families, enabling you to offer compassionate care and suitable therapeutic interventions.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is essential for this role.
• Strong interpersonal, communication, and collaboration skills to effectively engage with both patients and medical professionals.
• Proficiency with electronic health record (EHR) systems to maintain patient documentation.

• Two years of clinical social work experience in a healthcare or hospital setting is preferred.
• Familiarity with medical terminology and crisis intervention techniques.
• Experience in hospice or palliative care settings is highly desirable.
