Key Responsibilities
  • Perform ongoing assessments of patients to determine their mental health needs and tailor treatment plans accordingly.
  • Effectively communicate and collaborate with supervisors, direct care staff, family members, and other relevant stakeholders regarding patient progress and treatment efficacy.
  • Develop and provide a structured schedule of appointments, home visits, and follow-up assessments to ensure continuity of care.
  • Conduct home visits for in-depth evaluations, including chart reviews and staff interviews, to gather comprehensive data for service delivery.
  • Ensure the successful implementation of individualized treatment plans and report on their effectiveness during interdisciplinary team meetings.
  • Maintain meticulous records of patient progress and treatment outcomes to inform future care and ensure accessibility of information to authorized personnel.
  • Assist in organizing training sessions to equip fellow staff members with the necessary skills for implementing treatment protocols and documentation practices.
  • Actively engage with the mental health community and public resources to support ongoing development and improvement of client programs.
  • Provide continued support to clients via follow-up interviews, home visits, and progress assessments to adapt treatment plans as needed.
Patient Population / Client Focus

The LPC will work closely with diverse populations, including both adults and youth, addressing a variety of mental health concerns. You will be instrumental in providing tailored therapeutic support to individuals struggling with anxiety, depression, trauma, and relationship issues. By enhancing access to mental health services, your role will directly contribute to improving the overall well-being of the community we serve.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related field.
  • An active and unrestricted license as a Professional Counselor (LPC) in the state of Arizona.
  • A minimum of two years of counseling experience in a clinical or comparable setting.
  • A thorough understanding of ethical and legal standards in mental health care.
  • Strong interpersonal and communication skills, enabling effective relationships with a varied client base.
  • Demonstrated ability to collaborate effectively within a multidisciplinary healthcare team.
  • Proficient use of EMR software and related technology essential for documentation and service delivery.
Preferred Experience
  • Specialization in areas like family therapy, couples therapy, trauma-informed care, or substance use disorders is preferred.
  • Previous experience in providing therapy to a broad demographic, including underserved populations.
Compensation and Benefits
