Gotham Enterprises Ltd is searching for a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in providing exceptional care for individuals and families within our hospice center. Work Environment

This role is predominantly remote, allowing for flexibility while ensuring professional engagement with clients and family members. Although part of a larger organization operating from Sammamish, Washington, you will have the resources and support to effectively deliver services from the comfort of your workspace. Key Responsibilities
  • Conduct comprehensive evaluations and assessments targeted at identifying the psychosocial needs of clients receiving hospice care.
  • Develop and implement Individualized Family Service Plans (IFSP) collaboratively with clients and their families, tailoring approaches to meet unique needs.
  • Monitor and report on the progress and outcomes of care plans while actively participating in multidisciplinary team meetings to facilitate holistic patient care.
  • Provide one-on-one and family counseling, as well as emotional support, utilizing evidence-based practices to assist families during difficult times.
  • Ensure compliance with Federal and State regulations related to hospice care and social work practice.
  • Utilize a coaching model to educate families about the hospice experience and support them in navigating available resources.
  • Engage with healthcare providers and community organizations to advocate for the needs of clients and enhance care delivery.
Patient Population / Client Focus

Your primary focus will encompass individuals in hospice care, often dealing with terminal illness, allowing you to provide specialized support tailored to their unique circumstances. You will engage with families facing emotional and psychological challenges related to end-of-life care, emphasizing empathy and understanding within your interactions. The environment is sensitive and necessitates a compassionate approach to care that supports not only the patient but also their loved ones.

Qualifications
  • A Master's Degree in Social Work or a Doctorate of Social Work from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.
  • A valid driver's license for potential travel to client locations when necessary.
  • Strong ability to adopt various learning styles and treatment modalities to effectively meet individualized treatment plan goals.
