While this role is primarily remote, you will be an essential part of our collaborative hospice center-based team in Knoxville. You will engage with medical professionals, therapists, and patients through virtual means, ensuring continuity of care and teamwork. • Conduct comprehensive psychosocial assessments through interviews and direct observation, alongside a thorough review of medical records, to evaluate how emotional, social, and environmental factors affect each patient’s journey to recovery.
• Deliver counseling services to patients and their families, facilitating emotional support as they adjust to life changes and cope with the challenges of rehabilitation.
• Collaborate effectively with our multidisciplinary rehabilitation team to craft individualized treatment plans that align with each patient’s specific psychosocial needs.
• Guide patients in accessing vital community resources, such as support groups, vocational training, and housing assistance to aid in their rehabilitation process.
• Educate patients and their families on available community resources and post-rehabilitation services to ensure a smooth transition back into their daily lives.
• Participate in discharge planning discussions, coordinating with healthcare providers to facilitate effective transitions to home or other care environments.

Our patient population is diverse, ranging from individuals recovering from strokes and orthopedic surgeries to those with chronic neurological conditions. As a Licensed Clinical Social Worker, you will play a vital role in understanding the unique emotional challenges our patients face, thereby contributing significantly to their rehabilitation experience and overall well-being.

• A Master's degree in Social Work from an accredited program is required.
• Must hold an active state license as a Licensed Clinical Social Worker (LCSW).
• Excellent communication and interpersonal skills are essential for effectively engaging with patients and their families.
• A thorough understanding of adjustment disorders, grief responses, and coping mechanisms is critical.

• Previous experience in a rehabilitation or medical setting is preferred, providing valuable insight into the complex needs of patients.
• Demonstrable skills in collaboration within an interdisciplinary team will be highly regarded.

The salary for this position ranges from USD 75,000 to 100,000 per year, commensurate with experience and qualifications, plus a competitive sign-on bonus.
