Your role as a Licensed Mental Health Counselor involves a variety of responsibilities designed to support patient wellness and recovery:

• Engage in direct communication with physicians and other healthcare professionals to report on patient progress, needs, and concerns.
• Maintain meticulous and timely documentation of clinical and progress notes in accordance with departmental policies and healthcare regulations.
• Collaborate with a multidisciplinary health care team to develop individualized treatment plans, which include discharge plans based on comprehensive psychosocial assessments.
• Provide counseling and crisis intervention to patients experiencing emotional and behavioral challenges, utilizing evidence-based treatment methods.
• Assist clients with discharge planning and care coordination, ensuring they have access to necessary services throughout the region.
• Consult and engage with clients' families or support systems to discuss treatment plans and facilitate successful discharge transitions.

Qualified candidates must possess:

• A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology.
• Current licensure as an LMFT, LPC, LCSW, or equivalent, demonstrating your capability to practice independently.
• A minimum of three years of experience in an outpatient setting, with a proven track record of delivering effective mental health services.
• Strong ability to work autonomously, demonstrating self-direction and exceptional organizational skills.
• Proficient computer skills, including familiarity with Microsoft Office and electronic health record systems.

While not mandatory, experience with telehealth platforms and a background in applied evidence-based treatment modalities will enhance your effectiveness in this role. Familiarity with community resources and crisis intervention strategies is also a plus.
