This is a remote position, allowing you to deliver quality mental health services through our telehealth platform. You'll utilize cutting-edge technology to connect with patients and provide them with the support they need, regardless of their location. • Conduct thorough virtual assessments and screenings to evaluate the mental health needs of patients, creating tailored treatment plans that address their individual circumstances.
• Provide short-term individual counseling sessions and crisis intervention for patients facing anxiety, depression, trauma, and other mental health concerns.
• Lead engaging therapy workshops and support groups through our telehealth platform, fostering community and connection among participants.
• Collaborate closely with multidisciplinary teams, including physical therapists and outreach coordinators, to coordinate comprehensive care and ensure optimal patient outcomes.
• Engage in community outreach efforts aimed at increasing mental health awareness through webinar series, social media campaigns, and local community events.
• Maintain meticulous, confidential patient records in adherence to ethical and legal standards, using our electronic health record (EHR) system for documentation and communication.

Your primary focus will be on supporting individuals with a wide variety of mental health challenges, particularly those who are concurrently engaged in physical rehabilitation. You will work with our multidisciplinary team to ensure a holistic approach to patient care, addressing both mental and physical health needs. Our patient population reflects the vibrant diversity of San Jose, encompassing individuals from various backgrounds and experiences.

• Active state licensure as a Marriage and Family Therapist (LMFT), Professional Clinical Counselor (LPCC), Clinical Social Worker (LCSW), or Psychologist.
• A minimum of a Bachelor's degree in Counseling, Psychology, Social Work, or a related field; a Master's or Doctoral degree is strongly preferred.
• Demonstrable understanding of the unique mental health needs among individuals receiving physical therapy, particularly within a telehealth context.
• Exceptional communication and interpersonal skills to effectively navigate relationships with patients and collaborate within a multidisciplinary team.
• Familiarity with evidence-based practices and brief therapy models applicable in digital counseling services.
• Ability to manage a varied caseload and prioritize tasks efficiently in a fast-paced virtual environment.

• Previous experience providing telehealth services or virtual therapy, with specific knowledge of online therapeutic tools and methodologies.
• Experience working in a multidisciplinary team setting, particularly in a healthcare environment, promoting integrative treatment methodologies.
• Deep understanding of cultural competency and practices that resonate with our diverse patient demographics.

• Annual salary between $115,000 and $130,000, commensurate with experience and qualifications.
