Key Responsibilities
  • Conduct thorough virtual assessments and screenings to evaluate the mental health needs of patients, creating tailored treatment plans that address their individual circumstances.

  • Collaborate closely with multidisciplinary teams, including physical therapists and outreach coordinators, to coordinate comprehensive care and ensure optimal patient outcomes.

  • Maintain meticulous, confidential patient records in adherence to ethical and legal standards, using our electronic health record (EHR) system for documentation and communication.
Patient Population / Client Focus

Your primary focus will be on supporting individuals with a wide variety of mental health challenges, particularly those who are concurrently engaged in physical rehabilitation. You will work with our multidisciplinary team to ensure a holistic approach to patient care, addressing both mental and physical health needs. Our patient population reflects the vibrant diversity of San Jose, encompassing individuals from various backgrounds and experiences.

Qualifications
  • Active state licensure as a Marriage and Family Therapist (LMFT), Professional Clinical Counselor (LPCC), Clinical Social Worker (LCSW), or Psychologist.
  • A minimum of a Bachelor's degree in Counseling, Psychology, Social Work, or a related field; a Master's or Doctoral degree is strongly preferred.
  • Demonstrable understanding of the unique mental health needs among individuals receiving physical therapy, particularly within a telehealth context.
  • Exceptional communication and interpersonal skills to effectively navigate relationships with patients and collaborate within a multidisciplinary team.
