• Conduct thorough psychosocial assessments of patients and their families to understand their unique needs, strengths, and challenges.
• Develop and implement individualized treatment plans that reflect best practices for hospice care and address issues such as grief, depression, anxiety, and adjustment to illness.
• Facilitate therapeutic interventions, including counseling and support groups, to promote emotional well-being and ensure patients and families receive comprehensive psychosocial support.
• Maintain accurate and confidential patient records using our electronic health record (EHR) system to track progress and adjust care plans as needed.
• Collaborate with a multidisciplinary healthcare team, including physicians, nurses, and chaplains, to coordinate patient care and ensure a holistic approach to treatment.
• Stay informed about the latest research and developments in social work practices, particularly in hospice and palliative care.

The ideal candidate will work primarily with terminally ill patients and their families, providing essential emotional support and practical resources as they navigate the complexities of end-of-life care. The successful LCSW will address a diverse range of issues, including coping with grief, managing chronic pain, and facilitating communication among family members and caregivers.

• A Master's degree in Social Work from an accredited institution.
• Active LCSW (Licensed Clinical Social Worker) licensure in the state of Virginia.
• Demonstrated ability to work effectively in a remote team environment.
• Strong communication and interpersonal skills, with an emphasis on empathy and active listening.
• Ability to work independently and manage time efficiently in a remote setting.

• Previous experience in a hospice, palliative care, or healthcare setting.
• Familiarity with bereavement counseling and family dynamics during end-of-life care.
