Position Summaryย 

Sylvan Health is seeking a compassionate, motivated, and clinically experienced Nurse Practitioner (NP) to join our growing clinical team as a part-time W-2 employee. This role is ideal for an NP who is passionate about obesity medicine, enjoys collaborating withย referring Primary Care Providers andย Registered Dietitians, and is excited by the opportunity to help build an innovative, evidence-based medical weight management program.ย 

This is primarily a patient care role focused on delivering virtual medical weight management visits for patients using GLP-1 medications. The Nurse Practitioner will partner closely with Sylvan Registered Dietitians to provide comprehensive, coordinated care that supports both medication management and sustainable lifestyle change.ย 

Because this program is in itsย early stages, the ideal candidate is adaptable, collaborative, and energized by helping build and shape clinical workflows, protocols, and best practices.ย 

Responsibilitiesย 

  • Conduct comprehensive 40-minuteย initialย patient evaluations for medical weight managementย 
  • Provide 20-minute follow-up visits focused on medication management, titration, monitoring, and ongoing clinical supportย 
  • Evaluate patients forย appropriate GLP-1ย therapy and prescribe medications within scope of practiceย 
  • Monitor treatment response, side effects, laboratory findings, and overall patient progressย 
  • Collaborate closely withย referring Primary Care Providers andย Sylvan Registered Dietitians to coordinate individualized care plansย 
  • Educate patients on medication expectations, safety, lifestyle changes, and long-term weight managementย 
  • Maintainย timelyย andย accurateย documentation within electronic health record systemsย 
  • Follow Sylvan clinical standards and partner-specific workflowsย 
  • Participate in recurring team meetings and collaborate on program development and quality improvement initiativesย 
  • Contribute to the ongoing development of Sylvan's medical weight management program and clinical protocolsย 

Productivity Expectationsย 

  • Conduct a combination of 40-minuteย initialย visits and 20-minute follow-up visitsย 
  • Maintain a consistent patient panel and appointment scheduleย 
  • Meetings, administrative responsibilities, and no-show/late-cancel time are incorporated into the salaried structureย 
  • Deliver high-quality, patient-centered care while supporting access and continuity across partner organizationsย 

Requirements

  • Current Nurse Practitioner certification (FNP, AGNP, or Adult-Gerontology Primary Care NP preferred)ย 
  • Active, unrestricted Nurse Practitioner license in applicable states or willingness to obtainย additionalย state licensureย 
  • DEA registration (or ability to obtain)ย 
  • Experience managing patients with obesity or metabolic diseaseย 
  • Knowledge of GLP-1 receptor agonists and other anti-obesity medicationsย 
  • Experience with medication titration, monitoring, and patient educationย 
  • Strong clinical judgment and evidence-based decision-making skillsย 
  • Excellent communication and patient counseling abilitiesย 
  • Comfortable working independently in a remote telehealth environmentย 
  • Comfortable using electronic health records and telehealth platformsย 
  • Collaborative mindset with enthusiasm for working alongsideย referring Primary Care Providers andย Registered Dietitians in an interdisciplinary care modelย
