Job Description
This is a fully remote, full time, 5-day, 8-hour position (8:30 AM- 5:00 pm MST/PST required). An active and clear RN and APRN or Physician Assistant license is required. Candidates must be willing and able to consistently work within Pacific Standard Time (PST) hours.
A bit about this role:
This is an exciting opportunity for a compassionate and skilled Advanced Practice Provider (NP/PA) to provide virtual primary care through our innovative telehealth platform. As a Virtual Primary Care APP, you will deliver comprehensive, high-quality care while fostering strong, lasting patient relationships.
You will provide ongoing management of chronic conditions, preventive screenings, acute care visits, and medication management, all through a virtual care model. You'll also work collaboratively with a multidisciplinary team to coordinate in-person services such as lab work, imaging, and specialist referrals as needed.

Your Responsibilities and Impact will include:
  • Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions.
  • Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans to optimize patient health.
  • Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) within the scope of virtual care.
  • Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary.
  • Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety.
  • Provide health coaching, lifestyle modification guidance, and patient education to empower patients in managing their health.
  • Identify patients needing referrals, or home-based services, and facilitate appropriate coordination with specialists or community resources.
  • Utilize and provide feedback on our electronic health platform, contributing to its continuous improvement to enhance patient and provider experience.
  • Participate in clinical case reviews, team huddles, and interdisciplinary collaborations to ensure high-quality, patient-centered care.
  • Engage in continuous learning and improvement, staying up-to-date with best practices in virtual care and primary care management.

Required skills and experience:
  • Active Nurse Practitioner (APP) license in good standing in existing or future Devoted States or the ability to obtain licensure in multiple states within 90 days of hire.
  • RN and APRN licenses are active and in good standing or Physician Assistant Licenses are active and in good standing.
  • Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
  • Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.
  • Active BLS certification at time of hire.
  • Shift times may vary in order to align with standard business hours (9:00 AM - 5:00 PM) in the patient's local time zone. Flexibility is required. Additionally, weekend shifts may be required in the future based on evolving patient care needs.

Desired skills and experience:
  • Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.
  • Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus.
  • Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).

Salary range: $120k -$155k base range plus performance based bonus paid out quarterly or annually ($10k-$20k) for a total comp range of $120k-170k
