• Conduct thorough assessments of patients’ psychological and social needs to develop individualized treatment plans that address their unique circumstances.
• Implement and facilitate therapeutic interventions, utilizing evidence-based techniques to support emotional well-being and promote positive health outcomes.
• Maintain accurate and secure records in accordance with HIPAA regulations, ensuring confidentiality and thorough documentation throughout the process.
• Collaborate closely with interdisciplinary treatment teams, including physicians, nurses, and other health professionals, to ensure holistic care and comprehensive service delivery.
• Stay informed about the latest research and developments in the field to enhance therapeutic practices and provide cutting-edge solutions for clients.

In this role, you will work primarily with individuals and families navigating the emotional and psychological impacts of terminal illnesses. Our patients are often faced with grief, anxiety, and complex family dynamics, requiring a nuanced approach to care. You will specialize in trauma-informed care strategies to effectively support adolescents and adults dealing with a range of issues, including depression and anxiety.

• A Master’s degree in Social Work from an accredited institution.
• Current active LICSW license in the state of Vermont or the state you are practicing from.
• Demonstrated proficiency in therapeutic techniques, with a strong foundation in solution-focused and evidence-based practices.
• Exceptional communication, interpersonal, and problem-solving skills that foster effective client relationships and team collaboration.

• A minimum of three years of clinical experience in a hospital or hospice setting is preferred but not required.
• Familiarity with electronic health record (EHR) systems and documentation best practices.
• Prior experience working with diverse patient populations, demonstrating cultural competence.
