• Conduct thorough clinical assessments of patients to identify needs and develop personalized treatment plans that address their psychosocial challenges.

• Implement and facilitate solution-based therapeutic interventions tailored to individual patient circumstances, utilizing evidence-based methodologies.

• Maintain accurate and timely clinical records, ensuring compliance with legal and ethical standards while facilitating efficient patient management.

• Work collaboratively with a multidisciplinary team that includes physicians, nurses, and other healthcare professionals to coordinate care and support patient goals.

• Provide education and emotional support to patients and their families, guiding them through complex decisions and helping them access additional resources as needed.

• Engage in continuous professional development by staying informed on the latest mental health research and best practices, actively contributing to team discussions and training opportunities.

This position focuses primarily on patients in hospice care, requiring sensitivity and understanding of end-of-life issues. You will work with a diverse client population, including patients battling terminal illnesses and their families, assisting them in coping with emotional and psychological challenges while fostering a dignified transition through their care experience.

• A Master’s degree in Social Work from an accredited institution.

• Active and valid LCSW license in the state of Nevada.

• Expertise in solution-based psychotherapy approaches, preferably within a healthcare setting.

• Exceptional communication, interpersonal, and problem-solving skills, enabling effective collaboration and patient engagement.

• A minimum of two years of clinical experience in social work, ideally with a focus on hospice or palliative care settings.

• Experience working with diverse patient populations, particularly in understanding the unique challenges faced by those at the end of life.

• Familiarity with electronic health record (EHR) systems and documentation processes relevant in a remote clinical setting.

• Annual salary ranging from USD 80,000 to 100,000, commensurate with experience and qualifications.
