• Conduct comprehensive initial assessments of patients to gauge their mental health needs and develop tailored treatment plans aimed at improving their quality of life.
• Implement evidence-based therapeutic interventions, including individual and group psychotherapy, addressing various mental health conditions such as anxiety, depression, and bereavement.
• Engage in crisis intervention strategies during critical situations, providing immediate support and intervention to ensure patient safety.
• Maintain accurate and up-to-date patient records through our electronic medical record (EMR) system, documenting treatment progress, interventions, and outcomes effectively.
• Collaborate closely with physicians, nurses, and other allied healthcare professionals to devise and coordinate comprehensive care plans for patients.
• Participate actively in staff meetings and clinical supervision sessions, offering insights and feedback to enhance team effectiveness and patient care standards.
• Commit to ongoing professional development by keeping abreast of the latest research and innovations in mental health practice, integrating them into treatment approaches.

In this role, you will be serving a diverse patient population, primarily individuals receiving hospice care. These patients often face complex emotional and psychological challenges as they navigate significant life transitions. The focus will be on providing empathetic and culturally sensitive support that addresses not only individual mental health needs but also the emotional and spiritual aspects associated with end-of-life care.

• A Master’s degree in Social Work from an accredited program is a prerequisite for this position.
• A current and valid license to practice clinical social work in Pennsylvania is required.
• A strong background in providing psychotherapy, particularly to individuals facing grief, loss, or chronic illness, is essential.
• Demonstrated excellent communication, active listening, and interpersonal skills to build rapport with patients and their families.

The salary for this full-time position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
