Living Innovations is dedicated to being Rhode Island’s provider of choice for Shared Living, putting the people we support at the heart of everything we do and delivering exceptional services to the people served and their Home Providers. We are seeking a detail-oriented and self-motivated Registered Nurse to join our team as a Nurse Trainer. In this role, you will provide critical clinical oversight and documentation support for participants in our program, ensuring compliance with health standards and timely access to care.

This is a remote/field-based intermittent contract role (approx. 10–15 hours/week). The ideal candidate will be able to maintain a flexible schedule based on participant needs.

Responsibilities

  • Complete new participant home visits/assessments.
  • Supporting providers with medication/significant health changes of individuals.
  • Provides support and possible training around such topics such as diabetes, seizure management, and other specific clinical matters, as requested
  • Complete Annual Summaries and Medical Care Plans to ensure continuity of ISP funding.
  • Ensuring accurate documentation including file reviews and maintaining detailed medication logs.
  • Maintain a clear email trail with Coordinators/PMs.
  • Promptly confirm visit requests and schedule dates via email.
REQUIRED EDUCATION, EXPERIENCE, LICENSE, CERTIFICATION
  • High school diploma or equivalency.
  • Registered Nursing License in the state where services are provided.
  • Three years of registered nursing experience in social services or healthcare setting.
  • Valid state issued driver's license and proof of current vehicle insurance.
REQUIRED SKILLS AND KNOWLEDGE
  • Ability to make wellreasoned decisions, both independently and as part of a team.
  • Ability to use discretion in handling confidential information.
  • Ability to use effective communication skills with all stakeholders.
