Key Responsibilities

• Conduct thorough clinical assessments of patients to identify emotional, social, and environmental factors affecting their well-being.
• Create individualized treatment plans that utilize solution-focused interventions to address critical needs and enhance quality of life.
• Deliver evidence-based therapeutic interventions via teletherapy, ensuring that each session is tailored to the patient's unique circumstances.
• Establish and maintain accurate clinical documentation, adhering to state regulations and organizational standards for patient confidentiality.
• Participate in regular case consultations and multidisciplinary team meetings to track progress and refine care strategies.
• Educate and support patients and their families, providing resources and guidance to navigate emotional challenges during the hospice journey.
• Engage in ongoing professional development by staying abreast of emerging research and evidence-based practices relevant to hospice care.

Patient Population / Client Focus

This role is dedicated to serving patients receiving hospice care, often at the end of life. You will work with individuals facing complex emotional challenges, providing critical support and facilitating their transition while also addressing the needs of their families. Understanding the nuances of grief and bereavement will be fundamental to fostering an environment of comfort and support.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of New Jersey.
• A minimum of 2 years of clinical experience in a similar role, particularly in hospice, palliative, or end-of-life care.
• Proficient in evidence-based psychotherapy practices, with a focus on solution-based interventions.
• Strong interpersonal and communication skills, enabling effective engagement with patients and families.
• Ability to work collaboratively within a remote team, demonstrating flexibility and adaptability.

Preferred Experience

• Experience with grief counseling and bereavement support.
• Familiarity with electronic medical record (EMR) systems for accurate documentation and data management.
• Engagement in professional networks or continuing education focused on mental health and hospice care.

Compensation and Benefits

This position offers a competitive annual salary in the range of USD 300,000 to 500,000, commensurate with experience and qualifications.
