As part of a multi-disciplinary hospice team, you will collaborate with healthcare providers, including nurses, physicians, and therapists, to provide comprehensive support. Regular virtual meetings and check-ins will foster a collaborative team environment despite the geographical separation.

• Conduct thorough psychosocial assessments of patients through interviews, observational techniques, and review of medical histories to identify critical emotional, social, and environmental factors that may influence recovery and comfort.
• Deliver empathetic counseling and support to patients and their families, helping them navigate the complexities of end-of-life issues, adjustment challenges, and emotional distress.
• Collaborate actively with the hospice care team to develop individualized treatment plans that integrate psychosocial needs and contribute to the overall care strategy.
• Guide patients and families in connecting with community resources, such as bereavement support, vocational rehabilitation services, or psychosocial support groups, enhancing their access to necessary support systems.
• Engage in discharge planning processes, ensuring smooth transitions for patients moving from hospice back to community resources or home care.
• Facilitate family meetings to enhance communication, understanding, and involvement in care decisions during critical moments.

Your work will primarily focus on individuals facing terminal illnesses and their families. By understanding the unique challenges this population experiences, you will tailor your interventions to meet their emotional and psychological needs. Our patient population includes individuals with various conditions, requiring specialized care approaches that emphasize dignity, comfort, and emotional support in their end-of-life journey.

• A Master's degree in Social Work from an accredited program is required.
• Current active licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Vermont.
• Demonstrable ability to conduct thorough psychosocial evaluations and provide counseling tailored to patient needs.
• Strong verbal and written communication skills to effectively interact with patients, families, and the healthcare team.

• Experience working within rehabilitation or hospice settings is highly preferred.
• Knowledge of coping strategies related to grief and loss, and familiarity with palliative care practices.
• Experience with electronic health record systems to maintain accurate and timely patient documentation.
