• Conduct thorough psychosocial assessments to identify patients' emotional, social, and financial needs.
• Collaborate with interdisciplinary medical teams to ensure comprehensive care for patients, facilitating communication about the psychosocial aspects of health.
• Provide both individual and group counseling sessions focused on issues such as adjustment, grief, and coping mechanisms for patients facing health-related challenges.
• Connect patients with community resources and support organizations to facilitate continuous care post-discharge.
• Offer emotional education and resources to patients and their families regarding treatment options and available services.
• Participate actively in case conferences and hospital committees, contributing social work expertise to enhance clinic operations.
• Implement crisis interventions and provide immediate support to patients and their families as situations arise.
• Maintain accurate, clear, and timely documentation in line with healthcare regulations and organizational policies.

You will work with a diverse array of clients from various backgrounds, all facing distinct challenges related to their health and well-being. Our patient population includes individuals grappling with chronic illnesses, mental health conditions, and socio-economic difficulties. This role is pivotal in not only addressing their mental health needs but also in helping them navigate their healthcare journeys and often complex life situations.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of medical terminology, mental health disorders, and evidence-based crisis intervention strategies.
• Excellent communication and interpersonal skills, with demonstrated abilities to foster collaboration across various healthcare disciplines.

• At least two years of experience in a healthcare or clinical setting is preferred, illustrating a solid background in working with diverse patient populations.
• Experience with telehealth platforms and virtual counseling modalities is advantageous, given the remote nature of this role.
