Position Overview

The Adaptable Human Solutions Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team. This full-time position offers the unique opportunity to provide vital psychosocial support to seniors and their families through a remote work setting. The ideal candidate will bring a minimum of two years of experience in social work, especially within hospice care or geriatric environments, and contribute their expertise to enhance the emotional and mental well-being of clients.

Work Environment

This position is fully remote; however, you will coordinate closely with our hospice center located in Chattanooga, Tennessee. We emphasize a team-oriented approach, valuing the perspectives and contributions of all staff members. The standard work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evenings or weekends required depending on client needs. Flexibility in scheduling allows us to ensure continuous care and respond to urgent situations effectively.

Key Responsibilities
  • Conduct comprehensive assessments of seniors’ psychosocial needs, focusing on emotional, mental, and social factors that affect their well-being.
  • Collaborate with clients and their families to create tailored care plans that address specific goals and aspirations, ensuring a personalized approach to support.
  • Provide ongoing counseling and emotional support, guiding clients and their families through significant life transitions and challenges.
  • Advocate for clients' rights, ensuring access to community resources and services essential for their support.
  • Work collaboratively with interdisciplinary teams, including nurses and other healthcare professionals, to coordinate and enhance the care provided to clients.
  • Conduct regular check-ins and home visits to monitor clients' conditions, assess safety issues, and respond to any concerns raised by clients or families.
  • Facilitate communication among family members, helping to resolve conflicts and ensuring everyone involved in care is aligned.
  • Educate clients and families about available community resources and support programs to promote social engagement and improve overall life quality.
  • Maintain detailed documentation of client assessments, care plans, progress, and interactions using electronic health record (EHR) systems.
  • Stay informed about current best practices in social work and uphold ethical standards in all interactions.

This position focuses on providing support to senior clients facing emotional and mental health challenges, especially those dealing with terminal illnesses or significant life transitions. Qualifications
  • A Master’s degree in Social Work from an accredited institution.
