This role exists to ensure timely clinical assessment, coordinated care planning, and clear communication across the care team.Key responsibilities include completing OASIS/clinical assessments, developing and updating individualized plans of care, administering medications and treatments per orders, providing wound care and patient/caregiver education, coordinating with physicians and interdisciplinary partners, and completing accurate, compliant documentation within required timelines.Required qualifications/skills:- Current Arizona RN license; active BLS- Strong assessment, critical-thinking, and care-coordination skills- Home health documentation competence (OASIS preferred)- Valid driver’s license and reliable transportation- Professional communication and patient-centered approachIf you are ready to apply your nursing skills in a structured home-based setting, we encourage you to apply today.How Your Day FlowsYour weekday schedule runs Monday through Friday, 8:00am–5:00pm, with a planned route of in-home visits in and around Maricopa County.
