What Will You Do?

In this role, you will be responsible for the oversight and supervision of the interdisciplinary team of professionals, including nurses, social workers, therapists, and home health aides. The Clinical Team Manager oversees and supervises the day-to-day activities, ensuring quality client care is provided and monitors compliance with organizational standards and federal/state regulations related to health care delivery.

Responsibilities

  • Assigns new client admissions to RN Case Managers and oversees clinical self-scheduling processes ensuring equal distribution of client assignments and scheduling
  • Review staff documentation, including OASIS to ensure compliance
  • Perform one-on-one case conference reviews and mentor staff appropriately
  • Assists field clinicians as necessary
  • Assists with quality review as needed
  • Supervises, trains, assures adequate staffing levels and evaluates performance
  • Monitors quality of care provided and completion of charting
  • Holds staff accountable to goals

Education and Experience

  • Licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) in Michigan

  • Will also participate in the Manager on-call rotation with the other managers on the team
